District did not respond to requests in a timely matter

District notified parent that it contract to conduct the student’s independent transition assessment

District must provide training to Team Chairperson re: providing notice for independent evaluation requests and submit documentation to DESE.

District must send copy of Evaluation Consent form, meeting invite, and attendance sheet re: student’s transition assessment
