District did not provide evaluation consent form within required 5 day timeline  

Compliance:

(1) District was not required to provide a referral for special education evaluation

(2) IEP Team did not agree to an extended evaluation period and district proposed initial evaluation consistent with public health guidelines  

Corrective Action

District must conduct evaluation and convene IEP meeting as a priority when schools re-open. District must provide documentation that evaluation was completed and notice of IEP meeting.
