District imposed a blanket policy pausing all in-person initial evaluations and re-evaluations until end of school year 

NOTE: District acknowledges it paused all in-person initial evaluations and re-evaluations under a blanket policy during statewide COVID-19 closures. Only Rating Scales, or teacher A and B assessment forms completed.   

Corrective Action

District already submitted corrective action plan for conducting evaluations and re-evaluations.

District must submit a list including: names of students requiring an initial or reevaluation; types of evaluations completed for each student; copies of meeting invitations; and attendance sheets.
