Finding

Noncompliance:

School did not respond to issue raised in its local report to DESE and did not show it provided written notice of the intent to act or refusal to act, which can be done using DESE’s N-1 or N-2 form.

Corrective Action

School must provide a memorandum to staff that a parent may request a special education evaluation verbally and that staff must issue notice through a refusal to act (N-2) or an intent to act (N-1) as well as an evaluation consent form when agreeing to conduct evaluations. School must send DESE a copy by 10/6.
